Long Term Care and Retirement Home Pharmacy Service

 
  • Unequivocal access to a Consultant Pharmacist! Our Consultant Pharmacist will visit your facility at least once weekly, or when requested by staff.
  • Consultant Pharmacists assist facility in meeting all medication management-related MOHLTC compliance standards
  • Collaboration with nursing staff, physicians, and administration to modify policies related to medication management, programs and services.
  • Provision of a comprehensive medication reconciliation procedure, in accordance with Accreditation Canada guidelines.
  • Provision of detailed medication incident reporting, following ISMP and Accreditation Canada's guidelines.
  • Representation on various committees including Professional Advisory, Infection Control, Ethics, Pandemic Planning, Palliative Care, and Accreditation
  • Provision of a safety-engineered lancet and insulin pen needle program for both LTC facility and retirement homes, depending on their needs. A sharps management service will also accompany this service.
  • Provision of monthly newsletters which are available on our website www.roulstons.com. Topics are clinically relevant to both LTC facilities and Retirement Homes.
  • Assistance with the completion of the medication portion of Heat Risk Assessments for all residents in a LTC facility.
  • Provision of detailed monthly Drug Utilization Statistics and Therapeutic Classification Reports to suit a facility's requirements.
